Focal cortical elevation at the lower pole of the patella with development of a vertical cleft (periosteal sleeve avulsion fracture). Note minimal surrounding edema suggesting injury is not "acute". There is also edema in the tibial apophysis consistant with co-existent Osgood Schlatter disease.
