Classic findings of ovarian torsion on CT with a medialized (posterior to uterus) and enlarged ovary with surrounding fluid. The lead point of a primarily fat-containing dermoid cyst helps to increase the confidence of this diagnosis. The patient proceeded to a left ovarian cystectomy and detorsion, with final pathology of a mature cystic teratoma.
