Cystic adrenal neuroblastoma with liver metastasis is a rare condition. MRI revealed that the lesion was cystic in nature with the thick walls of the lesion showing contrast-enhancement. No solid components were detected in the lesion. Differential diagnosis in adrenal masses includes hamartoma and hemorrhage. Most cystic neuroblastomas have a good prognosis and show spontaneous regression.
