After the case discussion with the oncologist, as the patient was submitted for two previous core-needle biopsies in other service and there is a doubt about the origin of the tumor, we opted for both masses approach. 

As pulmonary mass biopsy could be complicated with a pneumothorax, adrenal mass biopsy was performed first. The procedure had no complications.

Pathological examination revealed lung cancer with adrenal metastasis.
