This patient presented for a routine follow-up radiograph following prior bilateral lung transplantation. Bilateral hilar clips and the configuration of the sternotomy wires (clamshell rather than midline sternotomy) can alert the reporting clinician to the history of prior lung transplantation. 

Minor scarring/post-inflammatory changes at the left lung base are thought to be related to the procedure and recovery period.
