The image findings are classic for xanthogranulomatous pyelonephritis (stage I). The patient underwent resection with histopathologic confirmation of mixed cellular infiltrates and lipid-laden macrophages. Differential diagnoses on imaging include renal replacement lipomatosis and chronic granulomatous infection from tuberculosis.
