Has this film been obtained PA or AP? How can you tell?
PA. The easiest clue is the scapulae. In an AP film the blades almost always project over the rib cage, whereas in a PA film the usually project lateral to the ribcage.
What abnormality do you detect?
Hazy opacity in the lower medial part of the right lung, with obliteration of the right heart silhouette. There is associated volume loss in the right hemithorax. This suggests RML collapse.
PA chest x-ray demonstrated some increase in density in the right lower zone, medially. The right heart border is somewhat ill-defined. The horizontal fissure cannot be identified.
