What is the differential?
Most likely represents nodal enlargement, especially as the hila appear to also be bulky. Other possibilities (if the hila were to be normal) would include a vascular anomaly (e.g. aneurysm), thymic mass, congential cyst, thyroid mass (unlikely).
At the level of the aortic arch, on the right, is a density projecting laterally from the mediastinum.
