The characteristic lung findings of cylindrical and varicose bronchiectasis with tree-in-bud configuration in the lingula, middle and lower lung lobes along with dextrocardia make primary ciliary dyskinesia the most likely diagnosis.

Note the relative sparing of the upper lobes.
