Shoulder arthrography is a technique used to instilinstill contrast into the glenohumeral joint to evaluate the joint components. It is usually performed under fluoroscopic guidance although the use of US has also been reported 1.

All patients proceed to cross-sectional shoulder imaging after contrast instillation. This is usually MRI but CT is sometimes performed if there are contraindications to MR or if there is high suspicion of an associated bony abnormality.

History

Although direct intra-articular contrast injection with MRI has been the gold standard for gleno-humeralglenohumeral evaluation. recently, recently there is evidence that intravenous gadolinium can also be used. This technique is referred to as indirect arthrography whilst the traditional method has been termed direct arthrography.

Indications

  • shoulder instability
  • to assess joint structures
    • rotator cuff
    • glenohumeral articular cartilage
    • glenoid labrum

Procedure

A variety of approaches have been described to cannulate the glenohumeral joint.
