IfInter-arterial course of the right coronary artery in an anatomical variation of the right coronary artery (RCA). If the right coronary artery (RCA) has an aberrant origin from the the left coronary sinus, it can have an inter-arterial course where it passes to the right between the ascending aorta and the pulmonary trunk. It is a rare variant of RCA anatomy.
It is described as malignant as the vessel can be extrinsically compressed by the two large arterial structures, leading to RCA territory myocardial ischemia, myocardial infarction or sudden cardiac death especially in the athletic young population.
The orifice is often 'slit' like due to it's oblique orientation which can also contribute to impairedimpaired RCA blood flow.
