The pyramidal lobe of thyroid is a normal variant. It is seen as a third thyroid lobe and is present in 10-30% of the population.
It is likely related to a remnant of the thyroglossal duct. It usually arises from the isthmus upwards along the midline or is shifted towards the left of the neck, and is usually located anterior to the thyroid cartilage. It can be involved in any diffuse thyroid parenchymal disease.
It is not uncommon to see it on routine thyroid ultrasound.
