Presentation

Previous history of the left upper quadrant penetrating trauma

Patient Data

Age: 50 years
Gender: Male

The margin of the left diaphragm is not visualized. There are also left lung base collapse and an air-fluid level.

Barium study shows intra thoracic herniation of the stomach through a left diaphragmatic defect with an hourglass sign.

Features consistent with left diaphragmatic rupture with partially gastric herniation.
