New onset atrial fibrillation

Frontal chest x-ray shows mild volumetric reduction of right lung, in association with dextrocardia and a tubular shaped opacity along right cardiac border.
Oblique incidence displayed that tubular opacity as a venous structure, arising from the right hilum, extending to the inferior vena cava topography. These findings are highly suggestive of Scimitar syndrome, a form of partial pulmonary anomalous venous return (PAPVC).
CT a confirmed the chest x-ray findings, along with right diaphragmatic herniation and some areas of reduced lung parenchymal attenuation, probably due to vascular perfusion disturbance.
Case Discussion
Scimitar syndrome is characterized by a hypoplastic lung that is drained by an anomalous vein into the systemic venous system.
