Septic sacroiliitis

Presentation

The patient presented with a history of atraumatic, sudden-onset, severe left buttock and posterior thigh pain with a fever of 2 weeks duration. Pain was described as constant, and symptoms were aggravated by all movement, especially the acts of standing, walking, and sitting. His medical history was unremarkable, except a neglected infected wound of the left foot since 1 and ½ month.

Age: 25 years
Gender: Male

Pronounced subchondral and bone marrow edema with enhancement around the left sacroiliac joint with effusion within the joint space as well as synovitis. The left iliopsoas muscle shows diffuse edematous infiltration with abscesses. Note also edema with contrast enhancement of the gluteal muscles and small abscesses within the gluteus maximus and piriformis muscles.
