Multiple gas-filled loops of bowel. The right-sided abdominal loop is mottled and in the right iliac fossa, there is pneumatosis. Appearances here of NEC. No perforation. No portal vein gas.

This case demonstrates features of necrotizing enterocolitis. While most often managed with supportive treatment, it is important to know the indications for surgery (such as free gas).
