Rathke's cleft cyst in childhood

Case contributed by Ammar Haouimi
Diagnosis almost certain

Presentation

Abnormal brain CT done for trauma. The laboratory investigations were unremarkable.

Patient Data

Age: 4 years
Gender: Male

The MRI sequences demonstrate a well-defined midline cystic lesion within the sella arising between the anterior and posterior pituitary. It displays a low signal on T1WI, high signal on T2WI with thin enhancing rim of surrounding compressed pituitary tissue. The pituitary stalk is of normal signal, thickness, and enhancement.

Case Discussion

This MRI feature in asymptomatic a patient is most consistent with a Rathke cleft cyst which is rare in childhood since the underlying substrate is congenital, lesions thought to enlarge throughout life.
