Presacral extra-adrenal myelolipoma

Case contributed by Michael P Hartung
Diagnosis almost certain

Presentation

Further characterization of pelvic mass.

Patient Data

Age: 70 years
Gender: Female

Lobulated presacral soft tissue mass with interleaving fat.

Case Discussion

CT-guided biopsy of the mass returned a diagnosis of either extra-adrenal myelolipoma OR extramedulallary hematopoesis, as both contain hematopoetic tissue on pathological analysis. Because the patient did not have any bone marrow abnormalities to support the diagnosis of EMH, myelolipoma was felt to be most likely.
