Presentation

Headache, ataxic gait.

Patient Data

Age: 65 years
Gender: Male

Extra-axial cystic lesion in posterior fossa, posterior to left cerebellar hemisphere, displays hypointense signal in T1W images and hyperintense signal in T2W images, exhibits CSF signal in FLAIR sequences, not restricted diffusion in DW/ADC.

It causes regional mass effect, compressing the left cerebellar hemisphere, slightly elevating the tentorium, and causes adjacent bone remodeling, marked thinning of left occipital bone.

Case Discussion

Extra-axial posterior fossa cyst of CSF signal intensity, not restricted diffusion, causing bone remodeling, suggestive retro-cerebellar arachnoid cyst.
