Splenic laceration - AAST grade IV

Presentation

Blunt abdominal trauma.

Patient Data

Age: 8 years
Gender: Female

Multiple splenic lacerations up to 5 cm in depth, extending to the hilum with large areas of devascularisation involving > 25%. No active bleeding seen on postcontrast images or pseudo-aneurysm. Extensive intraperitoneal effusion (mean density = 40 HU) in keeping with a hemoperitoneum. 

No other injury of the solid intra-abdominal organs.

Left pelvic kidney is noted.
