Chilaiditi syndrome: pseudopneumoperitoneum

Incidental finding oc chest radiography.

Patient Data

Age: 55
Gender: Male

There is gas between liver and diaphragm that rugal folds within the gas suggesting that it is within bowel and not free. In this case interposition of right-sided sigmoid make the sign.

Case Discussion

The findings are consistent with Chilaiditi syndrome. A rare disorder where a part of the colon, or sometimes intestine, is abnormally positioned between the liver and the diaphragm. 
Chilaiditi’s Syndrome is important because it can simulate pneumoperitoneum so also called pseudo-pneumoperitoneum .
