Bronchiectasis is defined as irreversible dilatation of part of the bronchial tree. This case shows a lower lobe predominance of bronchiectasis, mainly on left.

Lower lung predominant bronchiectasis, which is the most frequent pattern, is most commonly idiopathic. However, some cases have known causes, such as:

  • Recurrent Childhood Infectionsrecurrent childhood infections
  • Aspirationaspiration
  • Fibrotic Lung Diseasefibrotic lung disease
  • Rare Causesrare causes as: Bronchiolitis Obliteransbronchiolitis obliterans from posttransplantation rejection is a rare cause of bronchiectasis  associated with patchy mosaic perfusion, air trapping, and bronchiolar obstruction. And hypogammaglobulinemia can also be a cause of lower lobe bronchiectasis, most often caused by recurrent infections.

On this case bronchiectasis is of unknown cause, given the frustrating history in the medical recordrecords.
