RIght adnexal thin-walled cyst with T1 hyperintense and T2 heterogeneous shading signal content. Finding compatible with cyst with blood content, in first hypothesis endometrioma.

Pathological anatomy report (resumed and translated from Italian):

Macroscopic: whitish parietal flap 11x5cm with a superficial hematic suffusion

Microscopic: parietal flap lined by endometrial epithelium, supported by thin rhyme of cytogenic stroma.

Conclusion: Finding typical of endometriotic cyst

Case Discussion

Classic imaging features of endometrioma, whose diagnosis is supported by the shading sign, which consists of a predominantly low heterogeneous signal in T2-weighted sequences.

The cyst became complicated with ovarian subtorsion and was removed laparoscopically.
