Focal nodular hyperplasia (FNH)

Presentation

Right upper quadrant pain

Patient Data

Age: 25 years
Gender: Female

The ultrasound images demonstrate a mass within the segment IVb, iso-to slightly hyperechoic as compared to the normal liver. The color Doppler shows a central vascularization with spoke-wheel like centrifugal flow.

The previously described mass appears iso-to slightly hypointense on T1WI/T2WI, isointense on DWI with an ADC slightly lower than that of the normal liver. The postcontrast dynamic sequences show an intense early homogeneous enhancement on the arterial phase and isointense to the liver on portal venous phase with no central fibrotic scar seen.

Case Discussion

The ultrasound and MRI features in this young female patient are highly suggestive of a focal nodular hyperplasia (FNH).
