Lipomatous hypertrophy typically spares the fossa ovalis, and lies anterior to the f ovalisit. Since the fossa ovalis is spared, the lipomatous hypertrophy can take on a dumb-bell appearance (on short axis images).

It is secondary to hypertrophy of preexisting fat, and may be associated with increased epicardial fat.
