Presentation

Work-up of left hydronephrosis.

Patient Data

Age: 65 years
Gender: Female

Left kidney is malrotated with the hilum facing anterolaterally. Left upper pelvicalyceal dilatation draining into a renal pelvis at the lower pole where there is a narrowing. Left lower pole pelvicalyceal system mildly dilated with gentle taper to the ureter. Left lower pole simple cyst.

Normal right renal rotation. No right renal mass, calculus or hydronephrosis.

No solid renal mass. No marked left-sided renal cortical thinning. Symmetrical nephrograms and renal contrast excretion.

Case Discussion

This left kidney is malrotated, generally considered an anatomical variation. There is upper pelvicalyceal dilatation possibly from a calyceal narrowing, which was seen as hydronephrosis on a prior ultrasound.
