Masticatory muscle hypertrophy

Presentation

Bitemporal and cheek soft tissue swelling of chronic duration. Background history of headaches.

Patient Data

Age: 40 years
Gender: Male

There are bilateral temporalis and masseter muscle hypertrophy. There is also bilateral parotid gland hypertrophy.

Right frontal sinus hyperdense material may allude to non-invasive allergic fungal sinusitis.

The medial and lateral pterygoid muscles are normal.

Non-contrast CT brain is otherwise unremarkable.

Case Discussion

Facial and temporal pseudo masses were confirmed to be secondary to bilateral masseter and temporalis muscle hypertrophy further accentuated by the additional presence of parotid gland hypertrophy. In this instance, the medial and lateral pterygoid muscles are normal and uninvolved.

The 3D image alludes to the potential physiological appearance of an aggressive and /or angry person and the source of a potential "minotaur" syndrome.

The headaches may be due to a possible chronic bruxism and/ or due to the non-invasive right frontal sinusitis.
