Misplaced lumbar pedicle screw

Case contributed by Tariq Walizai
Diagnosis certain

Presentation

Right lower limb numbness after the post-op-free symptomatic period for lumbar spine fixation due to trauma.

Patient Data

Age: 50 years
Gender: Male

Straightening of the lumbar lordosis with multi-level end plate irregularities is seen.

Orthopedic rods and screws transfix the L3 down to the S1 vertebrae, of which the right-sided screw at the L3-L4 level is within the intervertebral space distally and more proximally traverses the outside of the neural exit foramen.

Case Discussion

The left-sided pedicle screw at the L3-L4 level is within the intervertebral disc space and on the outer aspect of the neural exit foramen, which is the most likely cause of the presentation.
