Pancreatic insulinomas

Diagnosis probable

Presentation

Repeated fasting hypoglycemia was always less than 45 mg/dL, with immediate relief of symptoms after the IV glucose.

Patient Data

Age: 50 years
Gender: Female

Two small, well-defined pancreatic nodules (body and tail), hypointense on T1 and T1 fat sat, iso- to slightly hyperintense on T2 and T2 fat sat, with restricted diffusion mainly of the nodule located in the pancreatic tail. Both nodules show vivid enhancement on the arterial phase with rapid wash-out on the portal phase. The nodule located in the pancreatic tail shows restricted diffusion.

Case Discussion

The clinical presentation and the MRI features are suggestive of an endocrine tumor of the pancreas (insulinomas).

Insulinomas are the most common sporadic endocrine tumor of the pancreas. On imaging, they are usually seen as small, well-defined hypervascular nodules that may be located anywhere in the pancreas.
