Presentation

Left upper quadrant mass partially imaged on chest CT.

Patient Data

Age: 50 years
Gender: Male

Large left upper quadrant mass with peripheral nodular enhancement that slightly fills in on delayed images. A thin stalk of attachment to the left hepatic lobe is noted. 

The appendix shows mural thickening and enhancement with surrounding fat plane smudging, in keeping with acute appendicitis

Case Discussion

Large pedunculate hepatic hemangioma with a thin stalk of attachment to the left hepatic lobe. 

It is important to keep this diagnosis in mind in order to avoid unnecessary biopsy or patient anxiety, as this may be initially quite alarming and mistaken for an exophytic GI neoplasm such as GIST.
