Presentation

Syncope, right shoulder pain and upper limb weakness.

Patient Data

Age: 70 years
Gender: Female

AP Chest X-ray

Large confluent right upper zone opacity.

Abnormal right first rib.

Cardiomegaly.

Patchy bilateral lower zone opacity obscuring the pulmonary vessels compatible with edema.

Destructive, expanded 8.5 x 7.2 x 6.9 cm tumor of the right first rib with mixed internal density and scattered areas of calcification, especially along the superior aspect of the mass. Bilateral basilar small effusions and atelectasis.

Cholelithiasis. Partially imaged hepatic hypodensity may represent a cyst.

Case Discussion

Biopsy diagnosis of plasmacytoma.

The patient started chemoradiation therapy 1 week post-discharge.

CT scan of the brain showed no acute intracranial hemorrhage, midline shift, or mass effect.
