Ependymoma. The extrusion through the foramen of Luschka is characteristic ("plastic ependymoma"). Medulloblastomas tend to present in children, and usually demonstrate some restricted diffusion (small round blue cell tumours). Choroid plexus papillomas usually enhance even more brightly, and are usually associated with hydrocephalus.

This case demonstrates fairly typical imaging features of an ependymoma. Tumor necrosis, which was focally noted in this ependymoma, is one histologic parameter used for grading ependymomas. There were no other histologic features associated with aggressive biologic behaviour in this sample. It should be noted that the correlation between tumor necrosis and biologic behaviour is not as stringent in ependymomas as in diffusely infiltrating astrocytomas.
