Presentation

SOB. Former mining community resident.

Patient Data

Age: 60 years
Gender: Male
x-ray

Extensive bilateral upper lobe fibrosis.

7cm right upper lobe and 4.3cm left perihilar masses. Extensive nodularity in the upper lobes.

Left apical bulla.

Heart size normal.

5.5 cm left perihilar and 6.9 cm right upper lobe masses.

Extensive nodularity and fibrosis in the upper lobes. Bilateral apical bulla, larger on the left.

Multiple small volume mediastinal nodes - some part calcified.

Case Discussion

Both the plain radiograph and CT studies show an advanced classical example of progressive massive fibrosis, a condition most associated with former, but not exclusively, coal mining communities in those exposed to coal dust as faceworkers.
