Pseudomyxoma peritonei

Diagnosis almost certain

Presentation

Gradually increased abdominal distension and discomfort.

Patient Data

Age: 70 years
Gender: Female

There is a very large amount of fluid attenuation material in the peritoneal cavity, which is causing distension of the abdominal cavity. This material causes scalloping of the surface of the liver.

Multiple sub-centimeter nodules (one of which is calcified) are seen in the mesentery, which could be either lymph nodes or secondary deposits. No definite omental caking is appreciated.

The parietal peritoneum is showing a nodular surface in the upper abdomen with the differential of metastatic deposits.

Case Discussion

CT findings are concerning for pseudomyxoma peritonei with omental and peritoneal metastasis and evidence of right hip arthropathy, as described above.

Peritoneal fluid cytology was positive for atypia. Mucinous tumors are the cause of this finding, although it is not known which organ the cancer has originated from in this patient.
