Painful shoulder movements

Patient Data

Age: 45 years
Gender: Female
mri

Hyperintensity in the superior labrum, extending from approximately 10 to 1 o’clock position. Subtle hyperintensity in the intracapsular segment of the biceps tendon, starting from its attachment at the glenoid and extending proximally for approximately 9 mm. 

Other findings:

Supraspinatus tendinosis with a partial-thickness articular surface tear of the anterior fibers at the critical zone, involving approximately 60-70% thickness.

An interstitial tear of the anterior/mid fibers of the supraspinatus tendon is also seen at the footprint, involving approximately 20-30% thickness.

Infraspinatus tendinosis is seen with a suspicious partial-thickness articular surface tear of the mid fibers at the footprint, involving approximately 10-20% thickness.

Case Discussion

Findings are suggestive of a SLAP tear. The extension into the intracapsular segment of the biceps tendon could very well represent a type 4 tear.
