Headache of 2 months duration which worsens upon straining and standing. No history of surgery, spinal procedures or trauma.

diffuse, smooth, pachymeningeal enhancement
distended (rounded) venous sinuses including the intercavernous sinus
cerebellar tonsillar descent (7 mm below the foramen magnum)
Case Discussion
These imaging findings, coupled with the typical history that this patient gave, are consistent with spontaneous intracranial hypotension.
