Vertebra plana or pancake vertebra is the term given when a vertebral body has lost almost its entire height anteriorly and posteriorly, representing a very advanced crush fracture. It can occur with a variety of settings, including:
- trauma
- osteoporosis

- proliferation of Langerhans cells and mature eosinophils.
- most common cause of vertebra plana in children.
- can present as: lytic lesion (more common), vertebraplana, eosinophilic granuloma. claasic: double contour (beveled edge). usually involves a single level.

- the most common primary pediatric primary malignancy.
- Loss of vertebral height and increased sclerosis are comm in the vertebral body.
- vertebral metastases
- multiple myeloma

- B symptoms and multiple levels involvement unlike LCH which commonly involves a single level.
- osteomyelitis
- Bacterial infection commonly causes disk destruction and vertebralbody end-plate erosions, unlike spinal tuberculosis spondylitis commonly causes bone destruction with somewhatrelative disk preservation -> which can cause vertebra plana. However, some form of vertebral end-plate irregularity, sclerosis, or destruction is usually present with spinal TB.
