Presentation

Right foot pain.

Patient Data

Age: 25 years
Gender: Female
x-ray

Small amount of periostitis along the medial aspect of the distal second metatarsal diaphysis.

Extensive bone marrow edema within the whole 2nd metatarsal diaphysis, solid periosteal reaction in the middle third and extensive surrounding edema.

Case Discussion

Stress fractures occur as a result of microscopic injuries. With repeated cycles of loading overtime, accumulation of injuries may lead to frank fracture. Most commonly, stress fractures occur in the lower limb, particularly tibia and metatarsals, or spine in individuals predisposed to repeated episodes of strain, such as military recruits or athletes 1.
