Bilateral congenital pseudoarthrosis of the tibia

Presentation

Severe leg bowing, fracture and gait abnormality

Patient Data

Age: 5 years
Gender: Male

There is anterolateral bowing of the tibiae at its distal third. The cortex is thinned at this point associated reduction in the diameter of the medulla. The tibiae demonstrate a segment of bone loss simulating the appearance of a joint at its distal thirds. A note is made of angulation at this point with cupping of the tibiae proximal to the absent section and sharpened narrow appearance of the bone distal to this mimicking the appearance of a joint.

The fibula is over-tubulated and deformed.

 

 

Case Discussion

Congenital pseudarthrosis of the tibia is a rare condition. Commonly, the disease becomes evident within a child’s first year of life but maybe not be discovered up to the age of adolescence1. Bilateral occurrence is very rare 2. Affectation of the fibula is seen in one-third of cases 3.

Congenital tibial pseudarthrosis is characterized by segmental osseous weakness, resulting in anterolateral angulation of the bone. The osseous dysplasia leads to a tibial nonunion and, because of tibial bowing and reduced growth in the distal tibial epiphysis, shortening of the limb occurs.
