Presentation

Left shoulder pain.

Patient Data

Age: 70 years
Gender: Male
mri
  • absence of the anterosuperior labrum in the 1-3 o'clock position (axial, sagittal)

  • thickening of the middle glenohumeral ligament (axial, sagittal)

  • mild edema of the acromioclavicular joint with narrowing of the joint space, and subchondral cysts

  • protrusion of the inferior border of the enlarged acromioclavicular joint onto the underlying supraspinatus muscle without impingement (no tendinopathy or tears)

Case Discussion

Buford complex is a congenital anatomical variant that could cause strain on the superior labrum muscle and biceps predisposing it to SLAP injury.
