Case contributed by Craig Hacking
Diagnosis certain

Presentation

Abdominal pain, vomiting and distension.

Patient Data

Age: 50 years
Gender: Male

Dilated air-filled loops of small bowel in the upper and left regions of the abdomen. In the left upper quadrant there is a dilated air-filled single segment of large bowel. Lack of bowel in the right iliac fossa. No pneumatosis or evidence of free gas.

Case Discussion

The findings are in keeping with a cecal volvulus causing of obstruction of the distal small bowel.

The patient went on to have a laparoscopy which confirmed a 'loop' type of cecal volvulus. The cecum was ischemic and didn't perfusion adequately upon reduction of the volvulus, so the surgeons performed a modified right hemicolectomy. The patient recovered well and was discharged within a week.
