Calcaneal stress fracture

Presentation

Heel pain.

Patient Data

Age: 40 years
Gender: Male
x-ray

Small lucency in the posterosuperior calcaneal cortex.

Prominent marrow edema through the calcaneus posteroinferiorly with a linear low T1 signal completely crossing the medullary cavity. Periosteal and soft tissue edema adjacent to the posteroinferior calcaneum.

Syndesmotic, lateral collateral and medial collateral ligaments of the ankle are intact. 

Sinus tarsi is unremarkable. Plantar fascia origin is normal. Normal Achilles insertion. No retrocalcaneal bursitis.

Anterior/extensor tendons are intact. Small effusion of the common peroneal tendon sheath. Small effusion of the tibialis posterior tendon sheath. Remaining flexor tendons are intact.

Case Discussion

The calcaneum is a common location of a stress fracture. In this case, the patient had a history of overuse having started running long distances regularly.
