Presentation
Patient to be submitted to a cervical spine MRI due a later evaluation related to trauma. On MRI safety check list he refers a previous history of ocular implant. MRI technician came to discuss the case. Previous brain CT available on the PACS.
Patient Data
CT images show bilateral small focus of calcification located adjacent to the medial orbital wall corresponding to a trochlear apparatus calcification.

Case Discussion
The trochlear apparatus of the eye is a cartilaginous structure with a synovium-lined sheath permitting unimpeded movement of the superior oblique muscle. Calcification in this location has an unclear cause, but is a benign condition. It is a common condition in patients more than 50 years old.
The discrimination between intra-orbital calcification and foreign bodies may be important for patients, like in this case, who will be examined with MR imaging.
