Presentation

Workup for microscopic hematuria. Known and operated case of colon cancer.

Patient Data

Age: 60 years
Gender: Male

A 20 mm cyst-like lesion in the upper pole of the right kidney contains a few small dependent calcified foci. On delayed post-contrast images, the lesion shows excreted contrast accumulation inferring calyceal diverticulum.

In addition, several subcentimeter simple cortical cysts are seen in the kidneys.

Post-operative changes are seen due to the right hemicolectomy.

Case Discussion

A calyceal diverticulum will appear similar to a renal cyst on early post-contrast CT images, although it may contain stones in the dependent part.

In the excretory phase, the cystic structure fills with contrast material due to communication with the collecting system, and the layering of contrast material is seen within. This helps differentiate the calyceal diverticulum from a renal cyst, which does not connect with the collecting system.
