Presentation

Painful swelling of the left distal thigh.

Patient Data

Age: 14 years
Gender: Male

The MRI sequences demonstrate:

  • abnormal signal of the femoral bone marrow of metadiaphyseal location, extending from the mid-third of the femoral shaft up to the distal metaphysis with aggressive osteolysis, extensive periosteal reaction, and thinning with irregularity of the cortical bone with a cortical break
  • periosseous heterogeneously enhancing soft tissue mass invading and displacing the surrounding muscles, mainly the vastus lateralis
  • epiphyseal extension of the tumoral process through the growth plate

The patient underwent an ultrasound-guided biopsy that confirmed the diagnosis of chondroblastic osteosarcoma.
