Chronic osteochondral defect with cystic change

The sharply demarcated subchondral cystic lesion in the talar dome in the medial aspect returning low T1 and high T2 signal intensity. There is a surrounding mild T2 high signal region likely edematous.
No fracture or joint effusion.

Features most likely represent chronic osteochondral defect with cystic changes (stage 2A of osteochondral injury staging).
