Presentation

Recurrent attacks of headache. Recent history of COVID-19 pneumonia.

Patient Data

Age: 20 years
Gender: Male

Midline small focus of signal abnormality is seen at the splenium of the corpus callosum. It shows mildly increased signal on T2WI and FLAIR with restricted diffusion. No post-contrast enhancement.

An incidental small intraventricular lesion is seen at the level of foramen of Monro, displaying almost intermediate signal on different pulse sequences without post-contrast enhancement suggestive of small colloid cyst.

Case Discussion

Cytotoxic lesions of the corpus callosum represent cell-cytokine response that is associated with different pathologies such as infection, malignancy, metabolic disorders and others. It usually affects the splenium and appears on MRI in the form of increased signal on T2, FLAIR and DWI due to cytotoxic edema.
