Denervation atrophy of left gluteus minimus and tensor fascia lata muscles

Presentation

Left hip pain.

Patient Data

Age: 65 years
Gender: Male

Fatty atrophy of left gluteus minimus and tensor fascia lata muscles with hyperintensity in gluteus minimus muscle.

Thickening and hyperintensity are seen in left superior gluteal nerve. 

Bilateral hip joints show reduced space, thinning/partial loss of articular cartilage, marginal osteophytes and paralabral ossification. Irregularity of acetabular labrum is seen, predominantly on the left side.

Incidental findings:

  • mild tendinosis of bilateral biceps femoris tendons seen
  • multiple ill-defined hyperintensities are seen in bilateral perianal regions with possible internal opening at approx. 4-5 o’clock position consistent with perianal fistula/sinus
  • a small cystic lesion measuring approx. 10 mm is seen in anterosuperior aspect of the right femoral neck – likely synovial herniation pit
  • note is made of bilateral small inguinal hernias

Case Discussion

Fatty atrophy of left gluteus minimus, tensor fascia lata muscles with hyperintensity in gluteus minimus muscle. Thickening and hyperintensity in left superior gluteal nerve to suggest neural edema/neuritis. Findings are consistent with denervation fatty atrophy. The possible cause of this would be lumbar disc herniations which were seen in lumbar spine MRI, done elsewhere.
