L4-5 left posterolateral disc extrusion with caudal migration, indenting the left subarticular zone and encroaching upon the corresponding exit foramen, exiting L4, and traversing L5 nerve roots.
L5-S1 foraminal disc-osteophyte complex encroaching upon the exiting L5 nerve root.
