Dysembryoplastic neuroepithelial tumor (DNET)

Diagnosis probable

Presentation

Epilepsy. Known case of scaphocephaly.

Patient Data

Age: 30 years
Gender: Male

Small well-defined lobulated left parietal intra-axial lesion of cortical location. It elicits a low signal on T1, high signal with a "bubbly appearance on T2, mixed-signal with partial attenuation of some of the "bubbles on FLAIR sequences. No calcification or hemorrhagic component seen on GE sequence. No surrounding edema or contrast enhancement. No restricted diffusion on DWI-ADC.

Abnormal narrow with cephalo-caudal elongation of the skull (known case of scaphocephaly).

Moderate ventriculomegaly is noted.

Case Discussion

MRI features of a well-defined cortical lesion with a "bubbly appearance" on the T2 sequence, most consistent with dysembryoplastic neuroepithelial tumor (DNET).

DNETs are most often located in the temporal lobe (over ~65% of cases) although all parts of the CNS containing grey matter are potential locations. 

At this stage, the histological proof is not available.
